Position Summary: The Medical Records Clerk is responsible for the accurate and timely management of patient medical records and healthcare documentation across the organization. This role supports clinical operations, compliance initiatives, payer audits, and continuity of care by processing medical record requests, coordinating hospital discharge documentation, and maintaining accurate records within the Electronic Medical Record (EMR) system.

The Medical Records Clerk serves as a key liaison between providers, hospitals, payers, internal departments, and external healthcare entities while ensuring compliance with HIPAA and organizational privacy standards.

Essential Duties:

  • Process incoming and outgoing medical records requests in accordance with HIPAA and organizational policies.
  • Review requests for completeness and verify appropriate authorizations prior to release of records.
  • Upload, scan, index, and maintain patient records within the EMR system accurately and timely.
  • Maintain confidentiality and security of protected health information (PHI) at all times.
  • Track and document record requests, releases, and follow-up activities.

Qualifications:

  • High School Diploma or GED required.
  • Additional coursework or certification in healthcare administration or medical records preferred.
  • Minimum of 1–2 years of experience in healthcare administration, medical records, or related healthcare support role preferred.
  • Experience working within an EMR/EHR system required.
  • Experience with health information exchanges such as Contexture preferred.
  • Experience supporting payer audits, chart reviews, or compliance initiatives preferred.
