Medical Scribe/ Medical Assistant

Job Title: Medical Assistant / Scribe / Chaperone

Department: Clinical Operations

Reports To: Supervising Provider and/or Clinic Manager

Job Summary:
The Medical Assistant (MA) / Scribe / Chaperone supports the provider by performing medical assistant duties, documenting patient encounters in real time within the electronic health record (EHR), and serving as a chaperone for the provider during all patient encounters. This role is essential to clinic workflow, documentation accuracy, patient safety, and provider support. The MA functions within their certified scope of practice and does not independently diagnose, treat, or make clinical decisions.

Essential Duties & Responsibilities:

Chaperone Responsibilities (Primary Requirement)

  • Remain present with the provider during all patient encounters, unless the patient explicitly declines chaperone presence
  • Serve as a professional witness during all examinations and patient interactions
  • Ensure patient comfort, dignity, and professionalism at all times
  • Document chaperone presence in the medical record per clinic policy
  • Respect patient preferences and immediately step out if a patient declines chaperone presence, documenting the declination appropriately

Medical Scribe Responsibilities

  • Document all patient encounters in real time within the EHR, including:
  • History of present illness (HPI)
  • Review of systems (ROS)
  • Physical exam findings (as dictated)
  • Assessment and plan (as directed by provider)
  • Enter orders, referrals, and documentation elements at the provider’s direction
  • Assist with timely and accurate chart completion to support same-day note closure
  • Utilize approved templates and smart phrases appropriately
  • Maintain strict HIPAA compliance and confidentiality

Medical Assistant Responsibilities

  • Room patients and obtain vital signs
  • Verify patient demographics, medications, allergies, and chief complaint
  • Prepare patients and exam rooms for provider evaluation
  • Perform MA-level clinical tasks within scope (e.g., injections, labs, EKGs, point-of-care testing) as directed by the provider and per clinic policy
  • Maintain exam room readiness, supplies, and infection control standards

Qualifications

Required

  • Current Medical Assistant certification (CCMA, CMA, RMA, or equivalent)
  • High school diploma or GED
  • Strong typing and computer skills
  • Knowledge of medical terminology
  • Ability to maintain professionalism and confidentiality at all times
  • Strong attention to detail and situational awareness

Preferred

  • Prior medical scribing experience
  • Experience working closely with a single provider
  • Familiarity with EHR systems (e.g., eClinicalWorks)
  • CPR/BLS certification

Scope of Practice Statement:
The Medical Assistant / Scribe / Chaperone performs duties within the Medical Assistant scope of practice and documents encounters at the direction of the provider. This role does not independently diagnose, treat, prescribe, or make clinical decisions.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: 32 – 38 per week
