Responsibilities

• Transcribe detailed clinical notes during patient visits, capturing vital signs, symptoms, diagnoses, and treatment plans with precision.
• Utilize Electronic Medical Record (EMR) and Electronic Health Record (EHR) systems to input, update, and manage patient information efficiently.
• Apply medical terminology accurately to ensure clarity and consistency in documentation.
• Support coding processes by assigning appropriate medical codes based on documented diagnoses and procedures, adhering to current coding standards.
• Maintain organized and up-to-date medical records, ensuring compliance with privacy regulations and institutional policies.
• Assist healthcare providers in reviewing and verifying documentation for completeness and accuracy before final submission.
• Demonstrate knowledge of anatomy and physiology to accurately interpret clinical notes and facilitate proper coding.

Qualifications

• Proven experience as a Medical Scribe or in a similar medical documentation role, preferably with coding responsibilities.
• Strong transcription skills with excellent attention to detail and speed.
• Familiarity with EMR/EHR systems used in various healthcare settings.
• Solid understanding of medical terminology, anatomy, physiology, and common medical procedures.
• Experience with medical coding (ICD-10, CPT) is highly desirable.
• Knowledge of medical office operations and record management practices.
• Ability to work efficiently in high-pressure environments such as emergency rooms or urgent care clinics.
