Job Title : Medical social worker

Job Role :

• Assess patients' emotional, psychological, and social needs upon admission or during hospitalization.
• Work closely with physicians, nurses, and other healthcare professionals to create an integrated care plan for patients.
• Assist with discharge planning, ensuring patients have appropriate resources, follow-up care, and support at home.
• Strong verbal and written communication skills for interacting with patients, families, and healthcare teams.

Qualification : MSW

Experience : Fresher
