Medical Social Worker

Job Summary
We are seeking a dedicated and compassionate Medical Social Worker to join our healthcare team. In this vital role, you will provide comprehensive psychosocial support to patients and their families, assisting them in navigating complex medical systems and emotional challenges. Responsibilities

  • Conduct thorough patient assessments to identify social, emotional, and behavioral needs, including mental health counseling and behavioral therapy.
  • Develop individualized care plans that incorporate counseling, case management, and resource coordination for diverse populations such as children, seniors, individuals with disabilities, or those receiving hospice or post-acute care.
  • Provide crisis intervention and mental health counseling, utilizing techniques like motivational interviewing and cognitive behavioral therapy to support patients through difficult situations.
  • Facilitate individual and group therapy sessions focused on behavioral health, addiction counseling, PTSD care, dementia care, or working with individuals with autism or developmental disabilities.
  • Collaborate with interdisciplinary teams to ensure compliance with HIPAA regulations and accurate medical documentation within electronic medical record (EMR) systems.
  • Assist with discharge planning, home & community care coordination, and assisted living arrangements to promote continuity of care.
  • Collect data for clinical research projects related to social work practices or patient outcomes to support quality improvement initiatives.

Qualifications

  • Bachelor’s or Master’s degree in Social Work (MSW) from an accredited program; licensure as required by state law.
  • Proven experience in hospital settings, inpatient units, nursing homes, outpatient clinics, or hospice environments.
  • Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
  • Experience working with diverse populations including children (special education), geriatrics (Alzheimer’s and dementia care), post-acute patients, or individuals with disabilities.
  • Proficiency in crisis management techniques and familiarity with EMR systems for data collection and documentation.
  • Excellent communication skills for intake interviews, diagnostic evaluations, case management, and psychotherapy sessions.
  • Ability to handle sensitive situations with professionalism while maintaining compliance with HIPAA standards.
