Lend A Hand Social Worker Job Posting: We have an exciting opportunity for a Social Worker (LSW) to join Lend a Hand, part of our growing care management program! Our Social Workers provide complex care management in the primary care setting. Through sensitivity to social drivers of health, understanding of the role behavioral health challenges often play in a patient's overall health, and deep knowledge of socioeconomic safety net systems and resources, the Social Worker helps patients overcome barriers and make meaningful progress toward health-related goals. If you are passionate about improving health outcomes and working as part of a patient centered team, consider joining us! ~ 40 Hours/week, Full-time, Day shift ~ Hybrid (up to 2 days/week remote after training is completed) Maine State License Licensed Social Worker (LSW) is required.
Summary:
The Social Worker – MHMG role provides professional social work services that support the goal of the Population Health program to deliver risk-based population management services to patients at all times during their tenure of care.

The base range is an estimate and does not reflect the full value of our total compensation package.
The pay range for this position is $61,256.00 to $82,867.20 per year.
Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
