Bachelor of Science with a major in nursing preferred. Associate degree in nursing is considered only with relevant experience and/or active matriculation in a BSN Program
Requirements

NYS RN License

BLS
-
2 years of medicine experience

Minimum 2 years RN experience

EPIC experience highly preferred
Essential Duties & Responsibilities
PATIENT CARE
-
Deliver safe, competent, evidence-based care using the Relationship Centered Care model.
-
Assess and evaluate patient needs with strong critical thinking.
-
Integrate relevant assessments/interventions in care delivery.
-
Collaborate with interdisciplinary teams on developing and implementing care plans.
-
Prioritize care including teaching, rounding, coaching, after-care planning, and delegation.
-
Educate patients/caregivers on plan of care, transitions, health promotion, and disease prevention.
-
Communicate professionally with patients, families, and care team members.
-
Manage assignments within Modified Primary Nursing and allocate material resources effectively.
PATIENT EXPERIENCE
-
Demonstrate key Relationship Centered Care practices (Admission Welcome, Bedside Shift Report, HELP, PHR, etc.).

Consider patient values, preferences, cultural diversity, and needs in all care.
-
Build and maintain therapeutic relationships with patients and families.
-
Use evidence-based practices to understand patient perceptions.
-
Initiate service recovery and escalate concerns when needed.
-
Demonstrate empathy, respect, compassion, and active listening.
-
Conduct Purposeful Hourly Rounding addressing the 4 Ps.
QUALITY & SAFETY
-
Practice safe, efficient, patient-centered care and escalate barriers.

Demonstrate clinical knowledge crucial for quality and safety.

Implement process improvement strategies using institutional policies.

Participate in peer and institutional review processes.

Support quality improvement projects, evidence-based practice, and nursing research.

Contribute to compliance and safety standards.

Apply high reliability principles to avoid high-risk behaviors.

Escalate safety hazards through chain of command.

Document thoroughly in institutional systems to support quality initiatives.
OPERATIONS

Perform charge nurse duties when assigned.

Coordinate with unit leadership to ensure proper use of supplies and inventory.

Escalate staffing or material resource shortages.
-
Demonstrate cost-effective, patient-focused resource use.

Document accurately per regulatory and legal standards.

Support patient throughput during shifts.
