What We’re Looking For
Trillium Health Resources has a career opening for a Member Engagement Coordinator to join our team!
The Member Engagement Coordinator is part of a dedicated team of professionals who serve as the front line of the organization who directly interface with members, family members, providers, and other stakeholders. The Member Engagement Coordinator has a focused role within the Member Engagement Unit to initiate interaction with Trillium members. They work with those members to engage and link them to services/supports provided through outreach efforts. This crucial role helps to guide members with making informed choices regarding but not limited to Tailored Care Management, Primary Care Providers and other identified resources to help them get on a path toward recovery and wellness that helps to improve outcomes for those members living with mental health, substance use, intellectual/developmental disabilities or traumatic brain injury.
On a typical day, you might:

Work in a Call Center environment taking inbound and making outbound calls to members determined to need outreach and engagement using data analytics.

Complete all documentation without delay, including call notes, transfer notes, complaint documentation, completion of demographic information contained in the Registration or the Screening, Triage, Referral form.

Promote satisfaction with the health plan and services through ongoing communication and timely follow-up on any concerns/issues.

Assists Trillium members by linking them to services, completing screenings, sharing information, providing technical assistance, answering questions, and handling and/or resolving complaints.

Ensures member engagement efforts are well documented for each attempt as needed to promote optimal health care and desired outcomes are achieved

Qualifications
Required:
-
High School diploma/GED and minimum six (6) years’ experience working with the adult mental health, developmental disabilities or substance use population. OR
-
Associate’s degree and minimum four (4) years’ experience working with the adult mental health, developmental disabilities or substance use population. OR
-
Bachelor’s degree and minimum two (2) years’ experience working with the adult mental health, developmental disabilities or substance use population. OR
-
Equivalent combination of education/experience Must have a valid driver’s license

Must reside within Trillium’s Catchment of North Carolina. Counties included: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.

Must be able to travel within catchment as required.
Preferred:

Experience providing crisis intervention or crisis response to individuals within the mental health, developmental disabilities or substance abuse population.
