The Mental Health Case Manager is responsible for providing a variety of services to individuals with serious mental health conditions. These services include one-on-one and group activities, help with managing symptoms in community, crisis support, and other tasks that promote recovery.
Ideal Candidates will be able to:
- Meet deadlines and ensure accuracy of paperwork
- Meet with consumers one-on-one in the community to support identified recovery goals.
- Facilities group to support identified recovery goals.
- Be able to work with children and adults.
- Provide support in crisis situations and be able to make decisions under stress.
- Observe and notify supervisor and treatment team of changes in behavior.
- Assist with using community resources
- Collaborate with treatment team to promote recovery.
- Have a flexible work schedule.
- Be reliable and responsible, with the ability to develop and maintain rapport with clients and staff.
Requirements:
- Bachelor in Human Services Field.
- Associates degree in Human Service field with two years of experience in mental health.
- Must possess a valid driver's license and a good driving record.
- Must be able to pass a random drug screen.
- Must be at least 21 years of age.
