Job Title Mental Health Case Manager

Work Schedule
This is a 37.5 hour a week position. The hours are generally Monday - Friday between 9:00 AM and 4:30 PM; however, hours may vary at times to meet program needs. A specific work schedule will be determined at the start of the position and can be adapted with approval or from or at the supervisors discretion.
Mental Health Case Manager – Job Description
A Mental Health Case Manager provides coordination and support services to adults with mental health needs (Members), helping them access resources, maintain stability, and work toward independence and recovery goals. This role is non-clinical and focuses on service planning, advocacy, and connection to care. Case Manager is responsible for their own Member caseload for a specific Service area(s) based on Member population and needs.
Key Responsibilities
- Conduct assessments and help identify Member needs, strengths, and goals.
- In collaboration with supervisor, create and implement monthly Services calendar based on Member needs and goals.
- Provide Member Transportation (using company vehicle) to Hedwig House In-Person Member Services.
- Provide Group Services at either a Hedwig House location or at other community locations.
- Provide Individual Services based on member needs and goals (such as: picking up individuals from work, taking individuals to appointments or shopping, assisting with bill paying, etc.).
- Assist as needed in the delivery of homemade and donated meals and food as well as groceries and other needed supplies to Members throughout Montgomery County.
- Connect Members to community resources such as housing, healthcare, employment, education, and benefits.
- Provide ongoing support, problem-solving assistance, and crisis intervention (non-clinical).
- Maintain accurate, timely documentation in compliance with agency requirements.
- Advocate for Members’ rights, access to services, and self-sufficiency.
- Other responsibilities as required
Required Skills & Qualifications
- Bachelor’s degree in social work, psychology, human services, or related field
- Strong communication, organization, and time-management skills
- Ability to manage a caseload and prioritize effectively
- Knowledge of community resources and social service systems
- Ability to build rapport with individuals from diverse backgrounds
- Experience in mental health, social services, or community-based programs
- Familiarity with recovery-oriented and trauma-informed approaches
- Valid driver’s license and reliable transportation
Work Environment
- Community-based settings, offices, or Members’ homes
- May involve travel within the service area and flexible scheduling
Agency Requirements:
- Follow policies and procedures of the agency, including administrative, quality assurance and personnel
- Perform related work as assigned, which may include lifting up to 15 pounds
- Maintain good attendance (daily meetings and other assigned tasks)
- Possesses required clearances
- As needed flexibility with work schedule to accommodate Member activities or meetings
- Maintain timely documentation of all work assignments and unusual incidents for risk management
- Routinely keep supervisor informed about attendance and job assignments
- Work cooperatively with peers
Job Type: Full-time
Pay: $43,000.00 - $46,000.00 per year
