Your Responsibilities Include:

Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.

Delivering dementia-informed, relationship-based care across the continuum of care.

Conducting comprehensive assessments and developing tailored care plans for older adults.

Managing chronic conditions and performing care with skill and empathy for patients and their families.

Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.

Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.

Maintaining consistency in documentation, follow-through, and patient communication.

What You’ll Bring
-
Active, unencumbered NP or PA license in the state you will be practicing.
-
Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).

Compassionate, patient-centered approach and strong clinical judgment.

Valid driver’s license and reliable transportation.

Pay Range for Posted Region: $140,000 USD - $200,000 USD
