Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine appropriate care plans.
- Provide primary care services such as immunizations, wound care, medication management, and health education tailored to individual patient needs.
- Manage acute conditions and chronic illnesses with evidence-based interventions, ensuring optimal patient outcomes.
- Collaborate with multidisciplinary teams to coordinate care plans, discharge planning, and follow-up procedures across various healthcare settings.
- Utilize electronic health record (EHR) systems like Epic or eClinicalWorks for accurate documentation of patient encounters and medical records.
- Engage in telehealth consultations to extend reach and improve access for remote or underserved populations.
- Educate patients and caregivers on disease prevention, medication administration, nutrition, and lifestyle modifications to promote long-term wellness.
Skills
- Proven experience with Epic EMR systems or similar EHR platforms such as Athenahealth for efficient documentation.
- Expertise in working with special populations such as geriatrics, assisted living residents, or individuals with developmental disabilities.
- Familiarity with home care procedures including tube feeding, catheter care, specimen collection/processing, and sterile techniques.
- Ability to perform diagnostic evaluations using laboratory testing or medical imaging when required.
- Excellent communication skills for patient education and interdisciplinary collaboration in fast-paced clinical environments such as Level I trauma centers or teaching hospitals.
- Competence in managing complex cases involving behavioral health issues or post-acute care needs like hospice & palliative medicine or memory care.
