MOBILE TREATMENT CASE MANAGER:

POSITION SUMMARY:The mobile treatment case manager provides services and functions as part of a team providing services to patients living in the community through mobile treatment or rehabilitation services. Candidates are able to provide a high level of complex interventions and work independently.

DUTIES AND RESPONSIBILITIES:

1. Provides experienced level of services that may include service linkage, assessment and planning of services, patient advocacy, monitoring of service delivery, patient outreach, and documentation.

2. Provides case management services for a caseload of eight to 15 patients.

3. Observe the behavior and physical condition of patient’s to assess mental health status.

4. Will manage and maintain medical records of assigned Mobile Treatment cases while participating on Medical Record Review team.

5. Provides crisis management, family supportive interventions and case management services.

6. Participate in 24-hour emergency coverage rotation and provide crisis intervention services.

7. Provides liaison services for patients with various community agencies.

8. Functions as part of a treatment team to serve and meet the needs of patients.

9. Participates in the development and implementation of patient treatment and recovery goals.

10. Documents all clinical activities in accordance with agency requirements.

11. Works schedule according to needs of agency.

12. Attends committee meetings as assigned.

13. Participates in ongoing clinical supervision according to agency policy.

14. Performs other duties as necessary.

Pay: From $20.00 per hour
