Position Summary:
As a Wound Care focused Nurse Practitioner, you will round within senior communities, delivering hands-on care and managing wound treatment plans. This role offers autonomy, flexibility, and the ability to build long-term patient relationships while making a measurable impact on outcomes.
Responsibilities:
- Provide on-site rounding and wound care services.
- Manage treatment plans, medications, and labs.
- Collaborate with physicians and care teams.
- Communicate with families and staff.
Qualifications:
- Active Certified Nurse Practitioner (CNP) license in Minnesota.
- Experience in geriatrics, primary care, or wound care preferred.
- Valid driver’s license.
- Strong passion for senior care.
