Nurse Care Manager

The Nurse Care Manager provides culturally competent, trauma-informed care to consumers through assessment, care coordination, advocacy, and person-centered planning. Under the direction of the Supervisor of Integrated Health Services, this role coordinates medical, behavioral health, and community services to ensure timely, appropriate, and goal-oriented care. The position supports the LifeWays Substance Use Disorder Health Home (SUDHH) model and upholds LifeWays’ mission, vision, and values.

Key Responsibilities

  • Engage consumers with empathy, professionalism, and a non-judgmental approach

  • Conduct assessments, reassessments, and person-centered service planning

  • Coordinate referrals and care within and outside the provider network, including Outpatient Psychiatry

  • Collaborate with psychiatrists, primary care providers, specialists, and community partners

  • Monitor caseloads and ensure ongoing medical necessity and appropriate levels of care

  • Support transitions across levels of care and life stages

  • Assist with crisis and after-hours planning

  • Provide services in person, by telehealth, and by phone

  • Perform care management functions for the SUDHH program

Required Education/Certification/Licensure
  • Nursing Care Manager I: Associate degree in nursing. Licensed as RN.
  • Nursing Care Manager II: Bachelor’s degree in nursing. Licensed as RN.
Preferred Education/Certification/Licensure
  • Master’s degree in human services related field.
  • Nursing Case Management Certification (CMGT-BC)
