Overview:

• Coordinate and manage care plans for long-term care residents
• Serve as a liaison between residents, families, and healthcare providers
• Ensure compliance with all regulatory and company standards
• Monitor resident outcomes and support care teams in achieving goals
• Provide guidance, education, and advocacy for residents and families
  
  
  

Requirements:

• Active RN or LPN license in the state of Ohio in good standing
• Minimum 3 years experience in long-term care, skilled nursing, or case management required
• Strong communication, organizational, and problem-solving skills
• Comfort working independently in a remote environment
