Position Summary

The Registered Nurse (RN) plays a key role in ensuring safe, person-centered, and compliant care for older adults receiving services through the Ohio Department of Aging (ODA). The RN is responsible for assessments, care planning, oversight of Personal Care Aides (PCAs), monitoring service delivery, and ensuring agency compliance with OAC 173-39-02 and 173-39-03 requirements.

The ideal candidate is compassionate, dependable, professional, and committed to maintaining the highest standards of care for aging adults in their homes.

Key Responsibilities (Aligned with ODA Requirements)

1. Clinical Assessments & Ongoing Monitoring

  • Conduct initial and ongoing in-home nursing assessments in accordance with the individual’s needs and ODA service requirements.
  • Identify changes in condition and promptly report findings to the care team.
  • Develop and update individualized care plans, ensuring they are person-centered and aligned with ODA standards.

2. Care Planning & Coordination

  • Collaborate with the interdisciplinary team, family members, and case managers to ensure accurate service delivery.
  • Participate in care conferences and maintain communication with ODA case managers when required.
  • Ensure care plans reflect the individual’s preferences, goals, risks, and support needs.

3. Oversight of Personal Care Aides (PCAs) / Homemaker Staff

  • Provide supervision, coaching, and training to PCAs to ensure compliance with OAC 173-39-03 service expectations.
  • Conduct supervisory visits, including in-home observations and documentation reviews.
  • Verify that PCAs follow the care plan, adhere to ethical standards, and maintain proper documentation.

4. Documentation & Compliance

  • Complete all nursing notes, assessments, supervisory forms, and care plan updates within required timeframes.
  • Ensure compliance with ODA documentation standards including:
  • verified service delivery
  • accurate time/mileage entries (if applicable)
  • incident reporting
  • adherence to agency policies and procedures
  • Support ongoing audit readiness for ODA, Medicaid, and internal quality assurance.

5. Health & Safety / Incident Management

  • Monitor for safety risks, abuse/neglect concerns, or environmental hazards; follow ODA-mandated reporting procedures.
  • Respond to emergencies and changes in condition per agency protocol.
  • Educate staff and clients on infection control, medication safety, and general health practices.

6. Training & Professional Development

  • Participate in required continuing education.
  • Provide input on staff training needs, particularly for personal care, infection control, and best practices for aging adults.

Qualifications

  • Current Ohio Registered Nurse (RN) license in good standing.
  • Minimum 2 year of clinical experience preferred (home care, geriatric, or community health experience is a plus).
  • Valid driver’s license, reliable transportation, and ability to travel to clients’ homes.
  • Strong communication and documentation skills.
  • Ability to work independently and maintain professionalism in all client interactions.
  • Must pass background check, BCI/FBI screening, and meet all ODA provider requirements.

Pay: $34.17 - $39.17 per hour

Expected hours: 5 – 30 per week
