Overview
The Nurse-Family Partnership Nurse Home Visitor provides comprehensive home visiting services to low-income, first-time mothers from early pregnancy through the child’s second birthday. The nurse works to improve maternal and child health, support positive parenting, and promote economic self-sufficiency using the evidence-based model developed by David Olds.
Responsibilities
- Provide regular home visits to enrolled pregnant women and families according to NFP guidelines.
- Assess maternal, fetal, and infant health and provide health education and counseling.
- Promote healthy pregnancy behaviors, including prenatal care, nutrition, and substance-use prevention.
- Support parents in developing positive parenting skills and child development knowledge.
- Screen families for mental health, domestic violence, and social service needs, and make appropriate referrals.
- Maintain accurate documentation and data entry in program systems.
- Collaborate with community partners including healthcare providers, social services, and early childhood programs.
- Recruit and maintain a caseload of 21 families.
- Participate in ongoing training, reflective supervision, and program evaluation.
- Maintain program fidelity to the Nurse-Family Partnership model.
Qualifications
Required:
- Bachelor of Science in Nursing (BSN) preferred.
- Registered Nurse (RN) license in the state of employment.
- Experience in maternal-child health, public health, pediatrics, or obstetrics.
- Valid driver’s license and ability to travel for home visits.
- Strong communication, assessment, and relationship-building skills.
Preferred:
- Public Health Nursing experience.
- Experience with home visiting programs or case management.
- Experience working with vulnerable or high-risk populations.
Work Environment
- Community-based position involving frequent home visits.
- Combination of field work and office documentation.
- Work with supervisor on remote and in office work requirement schedule

Pay: $35.00 - $39.00 per hour
