Nurse Navigator, Nursing Administration, Cheraw

• Utilize clinical, administrative, research and educational standards to facilitate the appropriate care of orthopedic patients including the Medicare Comprehensive Care of Joint Replacement (CJR) patients.

• Serve as a patient advocate and as a liaison to patients, physicians and other hospital departments.

• Assist in the development and growth of our total joint programs.

• Promote interdisciplinary collaboration and team work to achieve optimal clinical and resource outcomes within effective time frames.

• Maintain positive relationships with the surgery department, physician office staff, and surgeons.

• Develop, maintain, teach, and evaluate the pre-op joint classes.

• Participate in improving efficiencies with pre-admit testing, day hospital and physician office patient flows.

• Track quality improvement outcomes and trends for gaps or opportunities.

• Develop relationships with post-acute providers to ensure improved patient care after discharge.

• Perform other tasks, duties or responsibilities as assigned or needed for the effective, safe and efficient functioning and operation of the hospital.

Utilize lean methodology for continuous quality improvement.
