Ponos Mgmt, LLC ("Ponos") specializes in providing compassionate value-based care through a physician-driven, home- and mobile-based model. Our comprehensive approach integrates social, mental, physical, and economic solutions, addressing the unique needs of patients with chronic conditions such as sickle cell disease, ulcerative colitis, Crohn's disease, and severe rheumatoid arthritis. Our mission is to reduce emergency room visits and improve the quality of life for our patients through comprehensive, holistic coordinated care.
The Nurse Practitioner will have oversight of our unique membership population, with an clinical background in Hemotology, Rhumrheumatology and GI patients. This role will play a key leadership role on our clinical team with the following oversight:
· Patient Care: Provide comprehensive care to patients with conditions such as Sickle Cell Disease, Crohn’s Disease, and severe rheumatoid arthritis.
· Chronic Disease Management: Develop and implement care plans for patients, focusing on managing and improving chronic conditions.
· Patient Education: Educate patients and their families about their conditions, treatment options, and self-care strategies.
· Coordination of Care: Collaborate with other healthcare providers and partner organizations to ensure seamless care coordination.
· Conduct regular follow-up visits and telehealth appointments to monitor patient progress and adjust care plans as needed.
· Documentation: Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
· Participate in quality improvement initiatives to enhance patient outcomes and care delivery processes.
· Work closely with the Chief Medical Officer and clinical provider team to develop and implement clinical protocols and guidelines.
Qualifications:
· Certified Nurse Practitioner (NP) with a unrestricted license to practice in the state of Virginia.
· Minimum of 3 years of clinical experience, preferably in managing chronic diseases or working with the patient populations we serve.
· Strong clinical assessment and diagnostic skills.
· Excellent communication and interpersonal skills.
· Ability to work independently and as part of a multidisciplinary team.
· Proficient in using electronic health records (EHR) and other relevant software.
· Commitment to providing high-quality, patient-centered care.
· Bilingual skills are a plus.
Job Types: Full-time, Part-time, Contract
Pay: $130,000.00 - $140,000.00 per year
