Nurse Practitioner

To ensure quality and safe delivery of home care services in accordance with physician's orders. Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans. To assist in the provision and coordination of all home care services in accordance with Agency philosophy and established standards of home health and nursing practice.

• Conduct comprehensive assessments of hospice patients
• Collaborate with physicians, nurses, social workers, and chaplains
• Develop and manage individualized care plans
• Provide symptom management and palliative care interventions
• Educate patients and families about care goals and options
• Ensure documentation meets hospice and regulatory standards
• Participate in interdisciplinary team meetings and care planning

• Current Nurse Practitioner license in Virginia
• Board certification as an NP (AGNP, FNP, or equivalent)
• Minimum of [1–2+] years of hospice, palliative care, or geriatric experience preferred
• Strong communication and clinical decision-making skills
• Empathetic, patient-focused, and team-oriented
