At MD at Home, our Nurse Practitioners provide advanced primary care to homebound, chronically ill patients. Using a multidisciplinary, evidence-based approach, our team delivers consistent, compassionate care to patients who often have limited access to quality healthcare.

Responsibilities:

• Provide home-based primary care to chronically ill, high-risk patients
• Develop and implement individualized, evidence-based care plans
• Coordinate with interdisciplinary team members to optimize patient outcomes
• Collaborate with office-based physicians for guidance and support

Qualifications:

• Master’s degree from an accredited NP program
• National board certification as a NP
• State licensure as NP
• DEA licensure
• Successful completion of background check and drug screening

Pay: $120,000.00 - $150,000.00 per year
