Job Summary
We are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team, delivering high-quality patient care across diverse clinical settings. In this role, you will utilize your advanced clinical skills to assess, diagnose, and treat patients of all ages, ensuring comprehensive care that promotes wellness and recovery. Your expertise will support a variety of specialties, including primary care, acute care, geriatrics, behavioral health, and specialty areas such as gastroenterology, dermatology, and occupational health. Duties

  • Conduct thorough patient assessments including vital signs, physical examinations, and detailed medical histories using EMR (Electronic Medical Records) systems.
  • Diagnose and manage acute and chronic conditions across multiple specialties including internal medicine, geriatrics, pediatrics, emergency medicine, and behavioral health.
  • Prescribe medications and develop treatment plans in accordance with HIPAA regulations and state-specific scope of practice laws.
  • Perform diagnostic evaluations utilizing laboratory testing, sonography, medical imaging, spirometry, or other modalities; interpret ICD-10/ICD-9 coding for documentation and billing purposes.
  • Provide patient education on disease management, medication adherence, nutrition, and health coaching to promote wellness and prevent illness.
  • Participate in quality improvement and team meetings
  • Perform physical exams, VA Exam, DOT Physicals, order and interpret labs/imaging, and develop treatment plans
  • Manage chronic diseases (diabetes, HTN, COPD, etc.) and acute illnesses
  • Document patient encounters accurately in the EHR
  • Coordinate referrals and follow-up care
  • Collaborate with multidisciplinary teams in hospital settings—including ICU, trauma centers (Level I & II), neonatal units (NICU), PICU—and outpatient clinics to coordinate care plans.
  • Participate in clinical research initiatives or quality improvement projects to enhance patient outcomes and clinical protocols.
  • Ensure compliance with infection control standards and maintain accurate medical documentation in accordance with NCQA standards.

Qualifications

  • Valid Nurse Practitioner license with current certification; additional certifications in Basic Life Support (BLS) required.
  • Proven experience working in diverse healthcare environments such as hospitals (including trauma centers), nursing homes, assisted living facilities or outpatient clinics.
  • DEA registration
  • Certification required for Federal Motor Carrier Safety Administration (FMCSA) National Registry of Certified Medical Examiners (the clinic can help in registering)
  • VA Training required (the clinic will provide training)
  • Strong knowledge of EMR/EHR systems like Epic, Athenahealth or Cerner; familiarity with CPT coding for procedures and ICD coding for diagnoses is essential.
  • Experience with telehealth platforms for remote patient assessments and follow-up care preferred.
  • Demonstrated expertise in managing complex cases involving acute pain management, infectious disease care, hospice & palliative medicine or behavioral health issues.
  • Skilled in performing procedures including phlebotomy, venipuncture, catheterization, sterile processing techniques and physical examinations.
  • Ability to work effectively with diverse populations including children (pediatrics), seniors (geriatrics), individuals with disabilities or developmental delays.
