Locations: Chatsworth, Glendale, Temple City, Pasadena, Lynwood, Westminster/Long Beach, San Bernardino.

Position Type: Full-Time or Part-Time

Schedule: Onsite, part-time or full-time.

Compensation: $140,000 – $180,000+ earning potential

Position Summary

Maximize Health is a full-service medical group that provides high quality primary care for assisted livings. We are expanding services to Los Angeles and specialize in Cardiology, Podiatry, & Neurology. Will oversee 50-250 patients depending on location. Also, have telehealth.

What You'll Do

• Provide rehabilitation-focused evaluations and follow-up care for patients in assisted livings.
• Manage conditions such as:
• Stroke recovery
• Post-surgical orthopedic rehabilitation
• Traumatic injury recovery
• Mobility impairment and functional decline
• Neuromuscular conditions and spasticity
• Chronic pain management
• Develop and coordinate rehabilitation treatment plans with PT, OT, and speech therapy teams
• Round at assigned facilities multiple times per week
• Maintain documentation within facility EMR systems
• Participate in interdisciplinary care planning as needed

Qualifications

• Licensed Nurse Practitioner (NP)
• Interest or experience in assisted living.
• Must be willing to go on-site and to multiple sites.
• Strong communication and team collaboration skills

Compensation: Pay per note. 1099. Flat fee.

Pay: $135,384.11 - $163,043.23 per year
