Overview
We are seeking a dedicated, compassionate, and experienced Advanced Practice Registered Nurse (APRN) / Nurse Practitioner to join our growing Family Practice and Urgent Care team. The provider will deliver high-quality patient-centered care in both primary care and walk-in urgent care settings while working collaboratively with physicians and clinical staff.

The ideal candidate must be comfortable managing acute and chronic conditions, performing office-based procedures, handling urgent care visits independently, and providing preventive and wellness-focused care for patients of all ages.

Responsibilities

Key Responsibilities Clinical Responsibilities

  • Provide comprehensive patient care in both Family Practice and Urgent Care settings
  • Diagnose and treat acute and chronic medical conditions
  • Conduct thorough patient assessments including:
  • Vital signs
  • Physical examinations
  • Detailed medical histories
  • Risk assessments
  • Develop and implement personalized treatment plans utilizing evidence-based medical practices
  • Perform annual wellness visits, preventive screenings, and routine follow-up care
  • Manage chronic conditions including:
  • Type 2 Diabetes
  • Hypertension
  • Hyperlipidemia
  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Behavioral health conditions
  • Evaluate and treat urgent care complaints including:
  • Upper respiratory infections
  • Flu/COVID symptoms
  • Urinary tract infections
  • Minor injuries and lacerations
  • Sprains and strains
  • Skin conditions and allergic reactions
  • Gastrointestinal complaints
  • Dehydration and acute illnesses

Diagnostic & Treatment Duties

  • Perform physical examinations and urgent care evaluations
  • Order and interpret:
  • Laboratory testing
  • X-rays and imaging
  • EKGs
  • Diagnostic studies
  • Prescribe medications and treatment plans appropriately according to clinical guidelines and state regulations
  • Administer injections, nebulizer treatments, IV infusions, and in-office therapies
  • Perform minor office procedures including:
  • Suturing and wound care
  • Incision and drainage
  • Catheterizations
  • Airway management techniques
  • Splinting and injury stabilization

Documentation & Compliance

  • Maintain accurate, complete, and timely documentation within the electronic health record (EHR)
  • Utilize EHR systems efficiently while ensuring HIPAA compliance
  • Ensure charts are completed within office-required timelines
  • Document diagnoses, treatment plans, medication management, procedures, and patient education thoroughly
  • Participate in insurance quality measures, HEDIS initiatives, and gap closure programs
  • Collaboration & Care Coordination
  • Collaborate closely with physicians, specialists, medical assistants, nurses, and administrative staff to ensure quality patient care
  • Coordinate referrals, follow-up appointments, and continuity of care
  • Facilitate seamless patient transitions between urgent care, primary care, specialists, and hospital settings
  • Participate in team meetings, operational initiatives, and workflow improvement efforts

Pay: $100,000.00 - $130,000.00 per year
