About This Role

Nurse Practitioner exists to provide compassionate, high-quality primary care—serving as a trusted partner to patients as they navigate their health. This role blends in-office and virtual care, allowing you to meet patients where they are while delivering thoughtful, evidence-based care that prioritizes both outcomes and experience.

In This Role, You Will

• Serve patients across Arizona through both in-office and virtual visits, ensuring accessible, consistent care
• Be responsible for evaluating, diagnosing, treating, and following up with patients across a wide range of needs
• Partner with patients to create thoughtful, personalized care plans that support both immediate and long-term health

What You’ll Do

In this role, you will:

• Provide both in-office and virtual family practice consultations—evaluating, diagnosing, treating, and following up with patients across Arizona
• Manage acute and chronic conditions using evidence-based, patient-centered care
• Collaborate closely with Medical Assistants, a Medical Director, and the broader care team to ensure strong outcomes and a seamless patient experience
• Prescribe medications and develop treatment plans in alignment with state regulations and best practices Educate patients on preventive care, wellness, and long-term health management
• Maintain accurate, timely, and thoughtful documentation in our EHR system
• Stay current with evolving primary care and telehealth best practices

What We’re Looking For

We’re looking for someone who:

• Active Arizona Nurse Practitioner (NP) license, required
• Must be located in Arizona with flexibility to work both in-office and virtually
• Family Practice or Primary Care experience preferred
• Prior telehealth experience a plus
• Strong clinical judgment, communication skills, and a patient-first mindset
• Comfortable navigating EHR systems and telehealth platforms
