Key Responsibilities

• Provide comprehensive primary care to patients in their homes and facilities
• Develop and implement personalized treatment plans
• Review and utilize medical records to guide clinical decisions
• Communicate clearly with patients families and care teams
• Educate patients on conditions treatments and care plans
• Deliver compassionate holistic care aligned with patient goals

Qualifications

Required

• Active Nurse Practitioner license
• Minimum 2 years of primary care experience
• Strong clinical knowledge and decision-making skills
• Excellent communication and interpersonal skills

Preferred

• Experience in home health geriatrics or palliative care
• Strong ability to build relationships with patients and families

Work Schedule and Flexibility

• Flexible schedule typically between 7AM and 6PM
• Average of 40 hours per week
• No formal on-call requirement
• Occasional urgent patient-related communication as needed
