Key Responsibilities:

• Provide high-quality, compassionate care to patients in their homes and various facilities.
• Utilize comprehensive medical records and a strong support team to develop and implement personalized treatment plans.
• Communicate effectively with patients, families, and the care team, explaining disease processes and treatment options with empathy and clarity.
• Approach patient care holistically, respecting their desires and involving their families in decision-making.

What We’re Looking For:

• Licensed Nurse Practitioner with a minimum of two years in primary care (more experience is a plus!).
• Exceptional bedside manner, deep knowledge of primary care, and a personable nature.
• Ability to build strong bonds with patients and families, providing thorough explanations and compassionate care.
• Team player who is responsive, responsible, and has a strong work ethic.
• Someone driven to continuously improve, both medically and professionally.
• Palliative Care experience is a plus!

Working Hours and Flexibility:

• Enjoy a flexible schedule with hours typically between 7AM - 6PM.
• While there is no formal on-call requirement, be prepared to address occasional urgent messages about your patients.
