We are seeking an experienced APRN with infectious disease expertise to deliver high-quality, patient-centered care in skilled nursing and long-term rehabilitation settings. This in-person role involves managing complex infections and coordinating evidence-based treatment plans, and requires availability 5-7 days per week, including weekends and occasional after-hours on-call coverage.
Responsibilities:
- Provide infectious disease–focused patient care in skilled nursing, rehabilitation, and long-term care settings
- Perform comprehensive clinical assessments with emphasis on infection-related conditions
- Evaluate, diagnose, and manage acute and chronic infectious diseases, including respiratory, urinary, skin/soft tissue, and bloodstream infections
- Develop and implement individualized treatment plans aligned with evidence-based infectious disease guidelines
- Prescribe and manage antimicrobial therapies in accordance with state regulations and antimicrobial stewardship principles
- Monitor patient response to treatment, laboratory results, cultures, and antimicrobial effectiveness
- Conduct infection-focused admission, follow-up, and discharge evaluations
- Collaborate with attending physicians, infectious disease specialists, nursing staff, and facility leadership
- Support infection prevention and control initiatives within the facility
- Participate in antibiotic stewardship programs to reduce resistance and inappropriate utilization
- Complete accurate and timely documentation in the EMR/EHR
- Educate patients, families, and staff on infection prevention, treatment plans, and isolation precautions
- Participate in interdisciplinary rounds and initiatives to reduce infection-related hospital readmissions
- Ensure compliance with CMS, CDC, and facility infection control policies
Required Qualifications:
- Active Florida APRN license
- Active NPI number
- Minimum of 3 years of nursing experience (skilled nursing or post-acute care preferred)
- Current BLS and ACLS certification
- Strong clinical assessment, communication, and independent decision-making skills
- Experience with EMR/EHR systems (PointClickCare and/or Matrix preferred)
- Ability to work independently in a fast-paced clinical environment
- Knowledge of post-acute care and geriatric best practices
- Working knowledge of state and federal healthcare regulations, HIPAA, and patient safety standards
- Excellent written and verbal communication skills for documentation and interdisciplinary collaboration
- Compassionate, patient-centered approach to care
- DEA registration (preferred)
