Long Island Care Center is seeking a qualified Nurse Practitioner to join their clinical team in a skilled nursing and post-acute care setting.
Bilingual- Chinese (Mandarin and/or Cantonese speaker preferred)
Description:
The NP will provide comprehensive primary care, manage chronic conditions, and perform acute assessments to minimize hospital readmissions.
Key Responsibilities:
- Conduct comprehensive physical exams and assessments of sub-acute and long-term care residents.
- Collaborate with nursing staff, physicians, and multidisciplinary teams to develop and implement care plans.
- Manage acute and chronic illnesses, including ordering diagnostic tests, prescribing medications, and ordering treatments.
- Provide timely documentation within the EMR system.
Qualifications:
- Active, unencumbered Nurse Practitioner license (FNP, AGNP, or ANP).
- Prior experience in skilled nursing, post-acute care, or geriatric care preferred.
- Strong communication and collaboration skills.
