Position Summary:
We’re looking for a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team. The ideal candidate is passionate about root-cause medicine, eager to learn, and comfortable in a fast-paced, collaborative environment. You’ll play a vital role in helping patients take control of their health through individualized treatment plans and comprehensive care.

Key Responsibilities:

• Establish care with new patients and perform detailed Annual Wellness Exams
• Collect and document:
• History of Present Illness (HPI)
• Past Medical, Surgical, Family, and Social History
• Complete Review of Systems (ROS)
• Complete all components of the Annual Wellness Exam, including:
• Cognitive and depression screenings (e.g., PHQ-9)
• Functional and fall risk assessments
• Advance care planning
• Preventive screenings and medication review
• Order and interpret appropriate diagnostics:
• Blood panels, specialty labs
• Imaging (Ultrasounds)
• Functional scans (e.g., WAVI brain mapping, EKG)
• Educate patients on lifestyle, nutrition, preventive care, and functional treatment options
• Collaborate with our clinical and administrative team for a seamless patient experience
• Maintain accurate, timely documentation within the EMR (eClinicalWorks a plus)

Qualifications:

• Active California license as a Nurse Practitioner or Physician Assistant
• DEA license (or ability to obtain)
• 2+ years of clinical experience (functional medicine background a plus, but not required)
• Passion for integrative health and wellness
• Strong interpersonal and communication skills
• Detail-oriented and comfortable working independently in a team setting

Job Types: Full-time, Part-time

Pay: $65.00 - $80.00 per hour
