Job Description
We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to support facilities in Pittsburgh, PA. This role focuses on providing high-quality care to patients in skilled nursing and assisted living facilities, primarily within post-acute and long-term care settings.
- Part-time, per diem role (approximately 2–4 shifts per month)
- Hybrid model (in-person care + remote patient monitoring)

- Conduct comprehensive patient assessments and physical examinations
- Develop and manage treatment plans in collaboration with supervising physicians
- Review and interpret laboratory results and diagnostic data
- Provide patient and family education on health management and chronic conditions
- Document patient care accurately in electronic medical records (EMR)
- Coordinate care with interdisciplinary teams and facility staff
- Support ongoing patient monitoring and follow-up care
- Active and unrestricted NP/PA license in PA.
- Experience in post-acute, geriatrics, primary care, or related clinical settings preferred; open to candidates with strong clinical training and interest in learning
- Strong clinical judgment, communication, and interpersonal skills
- Experience with EMR systems and comfort with telehealth or remote patient monitoring (preferred)
- Ability to work independently while collaborating effectively with a care team
