Seeking a Nurse Practitioner or Physician Assistant (NP/PA) responsible for providing comprehensive primary care services to ambulatory patients. This role includes performing health assessments, managing common acute illnesses and chronic stable or complex conditions, developing and implementing treatment plans, and providing patient education and counseling. The NP/PA collaborates with physicians and other healthcare professionals to ensure high-quality, coordinated, and continuous patient care.
Provide primary care services, including patient histories, physical examinations, diagnostic testing, prescribing medications, and referrals in accordance with current clinical guidelines and protocols.
Requirements
- Current Registered Nurse (RN) license with Nurse Practitioner certification or current Physician Assistant license, as applicable.

Active and unrestricted state license to practice.
- Current CPR certification; ACLS and PALS certifications required.
- Minimum of one (1) year of experience providing primary care in an outpatient or ambulatory setting.
- Excellent verbal and written communication skills.
- Ability to work both independently and collaboratively within a multidisciplinary team.
- Ability to work effectively under pressure, assess patient needs, and make sound clinical decisions.
- Strong organizational skills with the ability to multitask and maintain attention to detail.
- Demonstrated experience in the delivery of primary care services.
- Sensitivity to and experience working with diverse patient populations.
- Three (3) or more years of experience in an outpatient or ambulatory care setting.
- Demonstrated leadership skills.
- Bilingual proficiency (Spanish or other languages) is a plus, but not required.
