We’re looking for a Nurse Practitioner who loves primary care and values building real relationships with patients. Our busy, well-established primary care practice in the Farmington Valley is growing, and we’re excited to add a compassionate, team-oriented APRN to our group. We pride ourselves on providing thoughtful, patient-centered care in a supportive and collaborative environment.

This is an outpatient position with a strong support team and an established patient base. You’ll have the opportunity to care for a diverse adult population, manage both acute and chronic conditions, and truly make a difference in your patients’ lives.

Responsibilities:

• Provide comprehensive primary care to adult patients
• Perform physical exams, diagnose and treat acute and chronic conditions
• Order and interpret diagnostic tests
• Develop and manage treatment plans
• Educate patients on preventive care and wellness
• Maintain accurate and timely documentation in the EMR

Requirements:

• Connecticut-licensed APRN (FNP or ANP)
• Primary care experience preferred, but strong new graduates are welcome
• Excellent communication skills
