Nurse Practitioner (Skilled Nursing)
Major Responsibilties
-
As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services
-
You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents
-
You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians

Evaluation of new-admission visits

Coordination of discharge visits
-
Monthly stability visits for long term residents
-
Evaluations for patients exhibiting changes in condition

As a SNF-ist, you will provide types of care, including but not limited to:

Advanced Care Planning
-
Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

Prescribe all necessary medications and treatments
-
Perform comprehensive physical examinations of patients
-
Develop and implement patient management policies and procedures
-
Facilitate referrals to other healthcare professionals and medical facilities
-
Communicate with attending physicians and/or specialists regarding patient care
Qualifications
-
Advanced Practice Licensure (APN) with a Florida license
-
Must have an active license and DEA number.

Previous experience in nursing or other medical fields.
-
Ability to build rapport with patients and their loved ones.

Ability to thrive in a fast-paced environment.

Excellent written and verbal communication skills.
-
Strong leadership experience with emphasis on teamwork in a healthcare setting.
-
Master’s degree required
