Compensation: $105,000 to $120,000 annually + flexible, provider-driven schedule.

Position Summary:

We are hiring a Wound Care focused Nurse Practitioner to support our growing footprint across the Twin Cities. In this role, you will provide mobile rounding care, advanced wound management, and chronic disease oversight within senior living settings. You will have the autonomy to manage your day, build your own patient relationships, and serve as a trusted clinical resource. Responsibilities:

Deliver on-site primary care and wound care services within senior communities.
Manage complex wounds including pressure injuries, diabetic ulcers, and post-surgical care.
Drive chronic care management and preventative health strategies.
Coordinate care with physicians, nursing staff, therapists, and facility leadership.
Reduce ER visits and hospitalizations through proactive interventions.
Communicate effectively with families, caregivers, and interdisciplinary teams.
Complete documentation, medication management, and lab coordination.

Qualifications:

Active Certified Nurse Practitioner (CNP) license in Minnesota.
Background in geriatrics, wound care, primary care, or post-acute care preferred.
Valid driver’s license and ability to travel between sites.
Strong ability to work independently and manage a field-based schedule.
Passion for serving seniors through relationship-driven care.
