OBGYN LVN Care Coordinator Lead (Full Time, Day)

At NorthBay Health, the OBGYN LVN Care Coordinator Lead is responsible for providing expanded duties as well as direct patient care in support of both providers and patients in an ambulatory care setting. They are highly engaged in serving our obstetric patients by facilitating patient centered care coordination services. The focus of care expands to the management of a population of obstetric patients from Orientation and IOB through delivery and involves coordination of care through the administration of our new OB classes, patient support services, referral management and specialty testing. Distribution of staff and patients may require reassignment to other nursing areas. The Licensed Vocational Nurse is responsible for monitoring and auditing clinic operations to ensure adherence to regulatory and professional standards.

PRIMARY JOB DUTIES

  • Provides direct patient care according per organizational policies and procedures.

  • Prepares patient for provider including vital signs and collection of clinical data.

  • Administers medications to patients within scope of practice to include specialty medications requiring a licensed professional such as Rhogam, Makena, Lupron.

  • Manages all refill requests within protocols of the department.

  • Initiates and manages all medication prior authorizations for a variety of specialty prescriptions exclusive to OB/GYN care.

  • Highly engaged in serving our obstetric patients by facilitating patient centered care coordination services. (i.e. specialized state regulated prenatal testing, MFM care, lactation support, specialty specific diagnostic testing)

  • Provides supportive guidance to obstetric patients via leadership of the Initial OB class, administration of EPDS and Perinatal 4P’s assessment forms and related risk identification tools.

  • Working within the established policy, provides diabetic meter training to obstetric patients in the absence of the RD.

  • Works as liaison between clinic RN and MFM team in the identification and management of high risk obstetric patients.

  • Uses protocols for managing OB/GYN patients effectively to obtain patient history and current complaint and facilitate/coordinate appropriate care.

  • In accordance with the above, documents all phone calls in the Electronic Health record using the SBAR format and correct medical terminology specific to OB/GYN care.

  • Responsible for staff training and compliance on Day one safety, Medication Administration, Trophon, Clinitek, glucometer, HgA1C and RALS, and sterile instrumentation processing.

  • Administers medications to patients including, oral and injections, as ordered by provider.

  • Assists provider and/or performs procedures in assigned area within scope of practice.

  • Documents patient encounters according to nursing policies.

  • Reviews preventative guidelines.

  • Keeps an inventory and prepares orders for supplies.

  • Actively participates in QI activities.

  • Presents a professional demeanor in appearance and attitude.

  • Performs audits to ensure adherence to regulatory and professional standards, including CPSP and ACOG guidelines for OB care.

  • Assists with the medication reconciliation/medication refill process relative to the electronic medical record.

  • Performs other duties as assigned.

  • Floats to other areas as needed.

LEAD JOB DUTIES

  • Serves as a clinical resource and mentor to OB/GYN LVNs and MAs, providing day-to-day guidance, coaching, and support to ensure consistent standards of care.

    • Assists clinic leadership with workflow coordination, assessment of staffing assignments, and patient flow management to optimize clinic efficiency.
    • Supports onboarding and ongoing competency validation of clinical staff, including training reinforcement and skills assessment.
    • Monitors adherence to clinical protocols, departmental policies, and regulatory standards, escalating concerns to leadership as appropriate.
    • Collaborates with providers, RNs, and leadership to identify workflow improvements and implement best practices in obstetric and gynecologic care.
    • Assists with coordination of schedules related to specialty clinics, procedures, and high-risk OB care to support continuity of care.
    • Participates in performance improvement initiatives, including data collection, audit review, and follow-up action planning.
    • Serves as a point of contact for complex clinical questions, patient concerns, and care coordination issues within scope of practice.
    • Supports communication between front and back office teams to ensure seamless patient experience and operational alignment.
    • Assists with supply management oversight, equipment readiness, and maintenance coordination to ensure uninterrupted clinical operations.
    • Reinforces infection prevention and safety standards and participates in clinic safety rounds and incident follow-up.
    • Collaborates with leadership in preparing for regulatory surveys, chart audits, and accreditation activities.
    • Assists in developing and updating clinic protocols, workflows, and educational materials related to OB/GYN services.
    • Promotes a culture of teamwork, accountability, and professional development within the clinical staff.
    • Provides coverage support and helps troubleshoot operational issues during high-volume or staffing shortage situations.
    • Participates in staff meetings and interdisciplinary huddles, communicating key updates and clinic priorities.
    • Escalates patient safety concerns, staffing issues, or workflow barriers to clinic leadership in a timely manner.
    • Models professional behavior and customer service excellence consistent with organizational values.
    • Supports leadership with tracking competencies, certifications, and required staff education.
    Performs other leadership-related duties as assigned.

Education :

  • Completion of accredited LVN Program.

Licensure/Certification:

  • Current California LVN license.
  • Current AHA or equivalent certification in CPR.

Experience:

  • Four years of experience in ambulatory care setting, OBGYN preferred.
  • Highly desirable skills: Public speaking for patient education / group classes and care coordination of high volume patient panels, leadership qualities.

Skills:

  • Current AHA or equivalent certification in CPR.
  • Demonstrated effective time management skills.
  • Must demonstrate and maintain current knowledge and skill in providing ambulatory nursing care as appropriate for patients in the following age groups: Geriatric, Adult, Young Adult, Pediatric and Neonatal. Bilingual Spanish highly preferred.

Hours of Work:

  • Based on business need. Monday through Friday between the hours of 8:30 and 5:30.

Compensation : $40 to $48.67 per hour based on years of experience doing the duties of the role.

Compensation Structure

NorthBay Health uses a structured compensation framework. Staff-level positions use a step-based system (Steps 1–5) based on years of directly related experience, with Step 5 representing 20 or more years of experience in the role. Manager level and above positions are paid a fixed annual base salary and are eligible for a variable incentive compensation plan. Physician compensation is structured based on specialty and role requirements.
