Position Summary
The ideal candidate will be responsible for preparing and sending patient-related correspondence—including care plans, physician orders, and clinical updates—to physician offices. This role requires strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks in a fast-paced healthcare environment.
Key Responsibilities

Prepare, review, and send care plans and other clinical documents to physician offices

Track and follow up on unsigned orders and outstanding documentation

Ensure all correspondence is accurate, complete, and compliant with agency and state regulations

Maintain organized records of all communications and documents sent/received

Communicate professionally with physician offices, clinicians, and internal staff

Assist with coordination of care to ensure timely approvals and continuity of services

Monitor deadlines and prioritize tasks to prevent delays in patient care and billing
Qualifications

High school diploma or equivalent (Healthcare administration experience preferred)

Minimum of 1-2 years of experience in home health or medical office setting

Strong organizational and time management skills

Exceptional attention to detail and accuracy

Proficiency in Microsoft Office and Kinnser

Experience with EVV or HHAeXchange is a plus
