On Call Registered Nurse

At The Care Team, we believe every day — and every life — matters. We’re looking for a compassionate, skilled Hospice Registered Nurse (RN) to join our growing team in a 7 on/7 off rotation — offering you the perfect balance of meaningful patient care and personal time.

What You’ll Do

As a Hospice 7on7off RN, you’ll provide high-quality, compassionate end-of-life care to patients and families in their homes and facilities throughout your assigned territory. This role requires autonomy, sound judgment, and a heart for service.

You will:

• Deliver direct nursing care and symptom management to hospice patients.

• Conduct assessments to monitor comfort, safety, and changing needs.

• Educate and support families, empowering them to provide care with confidence.

• Collaborate closely with physicians, social workers, chaplains, and aides to coordinate holistic care.

• Serve as primary on-call RN during your working week and ensure timely response to patient needs.

• Complete thorough and timely documentation in accordance with agency policy.

What We’re Looking For

• Licensed Registered Nurse (RN) in the state of Pennsylvania.

• Minimum 2 years of nursing experience preferred, hospice or home health a plus.

• Strong clinical and communication skills with the ability to work independently.

• Compassionate, patient-focused mindset with a commitment to excellence.

• Current CPR certification, valid driver’s license, automobile insurance, and TB testing per policy.
