SCHEDULE: Full Time, 8 HR Day Shift, 8am-4:30pm Monday - Friday
POSITION SUMMARY
The Outpatient Social Worker/B.S. for Transitional Care Management is responsible for supporting patients as they transition from inpatient hospital settings to outpatient care. This role focuses on improving patient outcomes, reducing hospital readmissions, and ensuring patients receive appropriate follow-up services. The social worker collaborates with interdisciplinary teams, patients, families, and community resources to facilitate smooth transitions and address psychosocial barriers to recovery. This job description outlines the essential functions and requirements for the Outpatient Social Worker in Transitional Care Management. Duties may be modified based on organizational needs and patient populations served.
KNOWLEDGE, SKILLS & ABILITIES
Substantial sitting, walking and standing. Light lifting (not to exceed 25 pounds), bending and pushing objects. Frequent exposure to patient care area. At times may be subject to high levels of stress. Requires considerable reading, mental effort.
