coordinates, manages, supports, and facilitates appropriate services to meet the needs of individual
clients.
Develop initial assessments of individual treatment needs by way of one-on-one sessions and use of any records that are sent by referral agencies to ensure quality clinical care and compliance with regulations
In collaboration with other resources, including but not limited to family members, agency representatives, and other agencies involved in the individual’s treatment, develop treatment plan goals for assigned individuals using behavioral health assessment tools and following guidelines established by regulatory and practice requirements
Maintain progress notes and progress reports by completing data assessment plan (DAP) documentation and modify treatment plans according to changes in individual’s status to ensure quality of clinical care and compliance with regulations
Provide clinically appropriate treatment, including individual and group counseling, to ensure treatment plan goals are met
Maintain a clinical caseload supporting sixty percent (60%) productivity, resulting in an average of twenty-four (24) billable hours each forty (40) hour work week to ensure financial sustainability of the program
Participate in program outreach activities, including but not limited to behavioral health screenings, community events, etc., to increase community impact, build connections with other referring providers, and increase program referrals to educate the community regarding mental health and to build census
Utilize individuals’ natural and professional supports as relevant, including but not limited to caregivers, medical providers, and supports coordinators, to assist individuals in meeting their treatment plan goals
Assist clients in adhering to treatment plans by setting up appointments, arranging for transportation to appointments, or providing support, including but not limited to phone reminders of upcoming appointments and creating outside referrals to other support agencies when appropriate, to ensure program participation and regulatory compliance
Maintain chart documentation, including but not limited to session notes, assessments, and treatment plans by completing identified documents to ensure adherence according to regulatory and payer requirements
Coordinate with other Elwyn clinical personnel, including therapists, psychologists, psychiatrists, and Certified Peer Specialists (CPS) relative to the individual’s overall treatment plan through tasks including but not limited to attending team meetings and providing consultation information, to support the overall outpatient program function and to support sister programs (e.g., New Visions, Partners in Recovery)
Attend internal team meetings and other outside meetings as directed by the Director or designee, including but not limited monthly staff meetings and dialectical behavioral therapy (DBT consultation team, with the goal to support the individuals served needs in reaching their treatment plan goals
Participate in weekly supervision with clinical director or designee by discussing clinical caseload and professional development to maintain license-eligible status
Perform other duties as assigned
Master’s degree required from an accredited university in a clinical mental health discipline, such as
psychology, counseling, clinical social work, psychiatric nursing, or marriage and family therapy
Demonstrated ability to work effectively as part of a teamability to work effectively as part of a
team
Demonstrated strong attention to detail
Demonstrated strong time management and organizational skills
Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
Demonstrated, intermediate experience with Microsoft Office applications, including Word, Excel, and Outlook
Must possess a current, valid driver’s license in the state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record, which will be assessed through a review of the Division of Motor Vehicles (DMV) record, in accordance with Elwyn’s Vehicle Operation and Driver’s Code Policy 905. For individuals licensed for more than two (2) years, the driving record will be reviewed for acceptability based on the past three (3) years. Additionally, employees required to drive as part of their job may need to successfully complete and pass an Elwyn Road Test before being authorized to drive.
Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR), preferred
Must be able to perform CPR, first aid and crisis intervention using agency trained protocols
