Position Summary
The Palliative Care Liaison (PCL) position will serve to support patient and family care by Coordinating completion of Advance Directives and ensuring that documents are scanned into the EHR or, when indicated referred to community resource to complete post discharge.
The PCL may assist in document completion and reinforcement of education to promote effective Advance Care Planning consistent with the mission of the organization.
The PCL will serve to support discharge planning process by communicating with and coordinating post discharge services and providers with oversight by the Nurse Case Managers and /or the Social Work Case Managers within the Case Management Department and may assist in collection of patient information as delegated by the RNCM and SWCM, as well as work to facilitate a transition plan that promotes a safe and effective discharge.
Then PCL or PCN will also perform other activities related to the palliative care and Advance Care Planning processes as requested.
Minimum Requirements
Associate's degree or higher in Human Services related field is required.
BS preferred.
Minimum of 2 years of clinical experience in hospice or palliative care preferred.
Two or more years - experience substituted for healthcare related BS.
Experience in case management, doctor's office, home health agency, DME agency, or health care field strongly preferred.
Pay Range
Pay is based on experience 34.74 - 57.33
