Palliative Nurse Practitioner – Part-Time & Full-Time Opportunities

We are seeking a Nurse Practitioner to join our growing palliative care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment.

Position Details:

• Part-Time: 20 hours/week (non-benefited)
• Full-Time: 32–40 hours/week (benefited)

Responsibilities:

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms
• Complete F2F encounters for hospice patients as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses
• Accurately document all encounters in the EMR
• Communicate effectively with patients, families, and other healthcare professionals

Requirements:

• Experience preferred but willing to train new Nurse Practitioners!
• Active Nurse Practitioner license in Wisconsin
• Strong clinical assessment, documentation, and care planning skills preferred
• Experience with eClinicalWorks (eCW) EMR preferred (willing to train)
• Excellent communication and teamwork abilities
