Part Time Case Manager

Overview

Connect With a Wish is a nonprofit working to improve the lives of youth in foster

care from birth to twenty five years. In 2023, we began our Transition to

Independence program. This program works with young adults aging out of foster

care to obtain a housing choice voucher and supporting them with wrap-around

case management to ensure successful independent living.

Responsibilities
Part-time case manager at Connect With a Wish.The position will

begin part-time at 25 hours per week at $18.00 an hour. The position will transfer

to full-time close to a year. Applicants should have a Bachelor's degree in any of

the following areas or a Bachelor's degree in an unrelated field with relative

experience: Include Social work, Human Services, psychology, healthcare. The

case manager will manage young adults who have been assigned a housing

voucher. Duties to include but not limited to: locate housing, voucher assistance,

managements of long term and short goals, and crisis management. Join our

Requirements
- Bachelor's degree in Social Work or related field preferred
- Proven experience in case management, crisis management, or related field ( Not Required)
- Knowledge of behavioral health issues and interventions
- Strong communication skills and ability to build rapport with diverse populations
- Certification in motivational interviewing is a plus
