Situated on a peaceful, wooded 26-acre campus , Nexus Neurorecovery Center – Conroe offers the RISE Program , a comprehensive neurological rehabilitation program for adults recovering from severe traumatic brain injury (TBI), stroke, spinal cord injury , and other neurological diagnoses .

This campus is also home to the Life Care Program , which provides long-term residential care for individuals who have sustained serious, life-altering injuries and require ongoing clinical and behavioral support.

Position Summary

The Patient Care Assistant (PCA) provides hands-on care, supervision, and rehabilitation support to adult patients recovering from neurological injuries or living with long-term care needs. Working under the supervision of licensed nurses and rehabilitation professionals, the PCA helps patients with daily routines, therapeutic activities, and behavioral support in a compassionate, structured environment designed to promote recovery and independence.

Key Responsibilities

• Provide direct care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, feeding, mobility, transfers, and grooming.
• Observe, monitor, and accurately document patient behavior, participation, and vital signs.
• Support therapeutic, rehabilitative, and recreational programming to encourage patient engagement and progress.
• Assist in implementing individualized care and behavioral support plans under clinical supervision.
• Maintain a safe, organized, and therapeutic environment, ensuring adherence to patient care protocols and safety procedures.
• Respond to crises or behavioral escalations using approved de-escalation and intervention techniques to maintain safety.
• Collaborate with nurses, therapists, case managers, and rehabilitation specialists to ensure coordinated and holistic care.
  
  

Licensure & Certifications

• De-escalation Training – required within 30 days of hire
• CPR / BLS Certification – required
