Job Overview

We are seeking a compassionate and highly organised Patient Care Coordinator to join our dedicated team. This is a Full-Time, remote position. In this vital role, you will be instrumental in supporting our mission to improve academic achievement, build character, and strengthen life skills for the youth we serve. You will act as a central point of contact, ensuring our members and their families have access to the resources and support necessary to thrive. 

Responsibilities

As a Patient Care Coordinator, your primary duties will include:
 

  • Serving as the main liaison between club members, their families, and internal or external support services.
  • Conducting initial assessments to understand the unique needs of each child and family, focusing on their health, well-being, and developmental goals.
  • Developing, implementing, and managing individualised support plans in collaboration with programme staff and families.
  • Coordinating with local community partners, healthcare providers, and social service agencies to connect families with essential resources.
  • Maintaining accurate, confidential, and up-to-date records of all interactions, care plans, and progress notes.
  • Proactively monitoring the well-being of members and providing ongoing support and guidance to their families.
  • Communicating effectively with all stakeholders to ensure a seamless and supportive experience for everyone involved in a member's care.
  • Championing the vision of our organisation, helping ensure every young person has a plan for their future and leads a healthy lifestyle.

Qualifications

The ideal candidate will possess the following skills and experience:
 

  • Proven experience in a similar role, such as care coordination, case management, or social work, preferably within a youth-focused environment.
  • Exceptional interpersonal and communication skills, with a natural ability to build rapport and trust with children and adults from diverse backgrounds.
  • Strong organisational and time-management skills, with the ability to manage a varied caseload independently from a remote setting.
  • A deep sense of empathy, patience, and a genuine passion for youth development and community support.
  • Proficiency in standard office software and experience with case management or client relationship management (CRM) systems.
  • A comprehensive understanding of local community resources available in the El Paso area is highly advantageous.
  • A degree in Social Work, Psychology, Public Health, or a related field is desirable but not essential.
