Patient Care Representative (Clinical & Facilities Support)

Position Summary: The Patient Care Representative (Clinical & Facilities Support) plays a dual role in ensuring exceptional care for patients seen at our office locations. This position serves as a key communication bridge between office staff, and patients or caregivers. The role involves handling clinical coordination by phone and supporting providers with administrative and care-related workflows. A strong foundation in clinical knowledge, effective communication, and critical thinking is essential. The ideal candidate will thrive in a team-based environment, be detail-oriented, and demonstrate empathy and professionalism when engaging with patients and staff. This position will be based at our Gibsonia office.

Expected Duties and Responsibilities:

Clinical Phone Support & Patient Triage

  • Serve as the first point of contact for clinical requests via phone, with a primary focus on supporting medication and test prior authorizations.
  • Accurately document patient inquiries using EMR dot text protocols and ensure timely, appropriate routing to providers or staff.
  • Facilitate communication between patients and providers regarding medication refills, post-discharge needs, test results, and related clinical concerns.
  • When necessary, schedule appointments related to medication refills, post-discharge follow-ups, or other clinically driven needs.
  • Utilize prior authorization platforms such as CoverMyMeds, Availity, Parachute, and internal logs to track and resolve outstanding requests.
  • Apply working knowledge of payer requirements, medication protocols, and office workflows to support timely patient care.
  • Collaborate with internal teams to ensure seamless coordination across departments supporting patient care.

Qualifications

Required:

  • High school diploma or equivalent
  • Minimum of 5 years of clinical experience in a healthcare setting
  • Proficiency in EMR systems and internal communication platforms
  • Demonstrated understanding of patient care coordination and clinical workflows

Preferred:

  • Medical Assistant Certification (CCMA)
  • Experience working in post-acute or facility-based care coordination
  • Customer service experience in a healthcare setting
  • Prior experience supporting providers in a remote or hybrid capacity

Job Type: Full-time

Pay: $18.00 - $21.00 per hour
