Patient Liaison – Victorville, CA
Salary: $70,000–$80,000/year
Territory: Field-based, travel required in the community

Calstro Hospice is seeking a compassionate, relationship-driven Patient Liaison to help families understand the Medicare Hospice Benefit and ensure a smooth transition into care. This role focuses on community outreach, referral partner engagement, and supporting the intake process.

What You’ll Do

• Educate patients, families, and facilities on hospice services
• Build and maintain strong relationships with referral sources
• Support seamless admissions by coordinating with internal clinical teams
• Conduct market outreach to drive growth
• Represent Calstro Hospice with professionalism and compliance excellence

What You Bring

• Bachelor’s in Marketing, Business, or related field (Master’s preferred)
• Strong communication, relationship-building, and problem-solving skills
• Ability to work independently, organize priorities, and collaborate with teams
• Hospice experience or knowledge of Medicare Hospice Benefit a plus
