Role Overview

One of our valued partners is seeking a full-time Patient Outreach and Enrollment Specialist that can work from Edge Campuses to provide essential clinical support and population health management. Reporting directly to the Director of Clinical Effectiveness, this position functions as a vital link supporting care teams to ensure patients, providers, and clinics are thoroughly prepared for high-quality, efficient visits.

The ideal candidate will possesses a healthcare educational background and at least one year of healthcare experience in fields such as medical assisting, care coordination, patient outreach, or population health, preferably within a Federally Qualified Health Center (FQHC) environment. This critical, non-hands-on role focuses heavily on pre-visit chart preparation—including identifying chronic condition care gaps and health maintenance items (HEDIS/UDS measures)—as well as conducting proactive outbound patient outreach to schedule preventive services and close loops on referrals or diagnostics. The position requires an organized, detail-oriented individual who can work independently while navigating electronic health records like NextGen.


Key Responsibilities

  • Patient Engagement, Enrollment, & Care Gap Support: Conduct proactive outbound outreach to promote healthcare participation, encourage annual wellness visits, and address gaps in care. Provide comprehensive enrollment and application assistance, including collecting required documentation, troubleshooting application issues, and conducting post-enrollment follow-ups.
  • Clinical Chart Prep & Appointment Scheduling: Ensure seamless scheduling, rescheduling, and tracking of appointments related to outreach initiatives. Utilize electronic health record components (including NextGen, Practice Management System, and QSI) to maintain accurate patient data, track care gap closures, and enter participant information.
  • Community Outreach & Partnership Development: Attend and assist with the development of community outreach events, such as health fairs, to promote organizational services and health insurance options. Collaborate with local organizations to establish referral partnerships and increase consumer access to a permanent medical home.
  • Reporting & Quality Tracking: Maintain rigorous tracking and follow-up systems for service applications. Prepare and submit daily or weekly productivity reports, monitor project details, and support monthly and yearly grant contract goals.
  • Compliance & Audit Readiness: Adhere strictly to Health Insurance Portability and Accountability Act (HIPAA) regulations and all local, state, and federal laws. Actively participate in Quality Assurance/Quality Improvement (QA/QI) initiatives, the Patient-Centered Medical Home (PCMH) model, and support audit events such as HRSA Operational Site Visits (OSV) and The Joint Commission (JC) reviews.


Requirements

  • Healthcare Experience: Minimum of one (1) year of experience working within a US healthcare setting. Candidates possessing advanced knowledge of the US healthcare system and/or direct experience handling US-based patient care will be highly preferred.
  • Education & Tech Skills: High School Diploma or GED Certificate. A background in healthcare is preferred, along with proven proficiency navigating electronic health records, specifically NextGen.
  • Communication & Relationship Building: Strong written and verbal English communication skills, including the ability to deliver presentations and effectively solicit feedback. Ability to build strong relationships and interface professionally with partnering healthcare organizations, local institutions, and a diverse public.
  • Operational Flexibility: This role requires individuals who can join immediately and work directly from EDGE campuses.
