Role Overview

Our valued partner is seeking a Patient Outreach and Enrollment Specialist to support community-based healthcare engagement, patient navigation, and enrollment assistance across a network of primary care health centers serving underserved populations.


This role is focused on increasing access to care by connecting community members with healthcare services, supporting enrollment into available health programs, and guiding patients through the full care journey from initial outreach to successful onboarding and follow-up care.

The successful candidate will play a key role in reducing gaps in care by actively engaging with community residents, coordinating outreach initiatives, and ensuring patients are supported in accessing medical, dental, and behavioral health services.


This position requires a strong community-facing mindset, excellent communication skills, and the ability to manage structured follow-ups, documentation, and multi-step enrollment processes in a high-volume, mission-driven healthcare environment.


Key Responsibilities

Community Outreach & Patient Engagement

  • Support outreach initiatives aimed at increasing awareness of healthcare services and available health programs.
  • Participate in community events such as health fairs, education drives, and enrollment campaigns.
  • Build relationships with local organizations and community partners to strengthen referral networks and improve access to care.
  • Promote preventive care, annual wellness visits, and early intervention programs to reduce gaps in care.

Enrollment & Patient Navigation

  • Provide end-to-end enrollment assistance, including application completion, documentation collection, and eligibility support.
  • Assist patients with navigating healthcare access, scheduling appointments, and resolving enrollment or access-related issues.
  • Ensure timely follow-up with applicants to support successful onboarding into healthcare programs.
  • Maintain continuity of care by guiding patients through post-enrollment steps and service activation.

Coordination & Communication

  • Coordinate with internal teams and external partners to ensure smooth scheduling, rescheduling, and follow-ups for patient appointments.
  • Communicate effectively with patients and stakeholders to provide updates, guidance, and support throughout the process.
  • Support development and distribution of outreach and educational materials for community engagement.

Data, Reporting & Documentation

  • Maintain accurate records of patient interactions, outreach activities, and enrollment progress.
  • Perform data entry and ensure integrity across tracking systems and databases.
  • Prepare regular reports on outreach activity, enrollment progress, and productivity metrics.
  • Support tracking systems that monitor patient progress from initial contact through successful enrollment.

Compliance & Operational Support

  • Ensure adherence to healthcare regulations, confidentiality standards, and organizational policies.
  • Support compliance with program requirements, audits, and quality improvement initiatives.
  • Participate in training programs, workshops, and professional development sessions.
  • Contribute to operational planning discussions and execution of outreach strategies.


Requirements

Experience & Education

  • High School Diploma or GED required.
  • Minimum 1 year of experience in healthcare outreach, community health, public health, patient services, or a related field.
  • Experience in community-based or primary care healthcare environments is strongly preferred.

Technical & Functional Skills

  • Strong ability to manage patient data entry, tracking systems, and documentation workflows.
  • Comfortable handling structured enrollment processes, scheduling systems, and follow-up coordination.
  • Basic understanding of healthcare access programs, patient navigation, or enrollment support services is preferred.

Communication & Interpersonal Skills

  • Strong verbal and written communication skills with the ability to engage diverse community populations.
  • Ability to build trust-based relationships with patients, community partners, and internal teams.
  • Comfortable delivering basic information and guidance in a clear, empathetic manner.

Core Competencies

  • Strong organizational and time management skills with attention to detail.
  • Ability to manage multiple tasks in a fast-paced, high-volume environment.
  • Problem-solving mindset with ability to support patients through multi-step processes.

Additional Preferences

  • Bilingual English/Spanish is highly preferred.
  • Familiarity with underserved communities and barrier-to-care populations is a plus.
